The Career Education Dept. has made available several required forms for students. Missing forms may result in hours not being credited.

• WEX12 Student APPLICATION Package
         ~ Available to students to download and & print

• Work Experience Placement Request Form 
         ~ Students complete to request a community-based placement.
         ~ Submit to your Work Experience Facilitator

• WEX12 Training Plan
         ~ To be completed by WEX12 student and signed by all parties.
         ~ Required for Paid OR Unpaid work placements

• WEX12 Work Term Report
         ~ To be completed by students following each work placement
         ~ Excludes 1-day job-shadows

• WEX12 Work-Based Training Log
         ~ For students to keep track of their placement hours throughout the school year

• Workplace Safety Assignment - Part 1 (ALL Career Programs)
         ~ To be completed by students BEFORE placements (Paid OR Unpaid) occur 

• Workplace Safety Assignment - Part 2: Workplace Bullying & Harassment (ALL Career Programs)
         ~ To be completed by WEX12 students BEFORE placements (Paid OR Unpaid) occur

         ~ To be completed by student and employer. Required for EACH placement.

• Employer Feedback Form (ALL Career Programs)
         ~ Students are responsible for having an Employer Feedback form completed for EACH placement

• Student Reflection Form (ALL Career Programs)
         ~ To be completed by student following a placement
